    2. Appointment updates with hours, directions and what to bring to their appointment..
    3. Patients can always text us about their questions.

       

      We do not share or sell SMS opt-in, or phone numbers for the purpose of SMS.

    • Phone number as a way to get in touch with you in order to address your providers concerns.

  • Referral Form: The data collected through the referral form is used to facilitate the referral process between healthcare providers and our clinic. This information is handled in compliance with HIPAA regulations to ensure patient privacy and data security.

  • Text-Message (SMS ) opt in on the referral form (Optional)

    • 1) To let the Patient know we’ve received a referral for them from the provider.

      2) Appointment information such as address, time, and what to bring to their appointment.

      3) To allow 2-way communication between the Patient and New York Sleep Disorder Staff regarding any concern.
